Travel Health

To help us offer the appropriate advice, you need to call at reception and ask for 'travel questionnaire' forms to complete.

Please note that nurse needs these forms completing prior to any contact with the patient.

Our practice nurse Karen Moore is a qualified travel nurse and will look at current recommendations and guidelines for your destination.

Please ensure you leave a contact number on the form so that Karen can contact you to arrange an appropriate appointment for you to come in and discuss further with her.

It is important to get the travel questionnaire form as early as possible - at least 6 weeks before you travel.

please note:

  • Some travel vaccines have to be ordered as they are not a stock vaccine.
  • Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
  • Waterloo House Surgery is a registered yellow fever centre.

Nurse will discuss your travel health needs in the appointment.
